Horizon Leisure Centres 12 week exercise referral scheme at Havant and Waterlooville is a structured course of physical activity. The scheme is in partnership with Havant Borough Council, The NHS and Macmillan Cancer Support.

Participants are referred to the exercise referral scheme through a health professional or GP where physical activity can help to:

Decrease body fat and body weight

Increase bone density

Help lower blood pressure

Help improve cholesterol levels

Help stabilize blood sugar levels

Improve lung function

Improve mood

Reduce stress and anxiety

Increase energy levels

Improve mobility

The key to enjoying physical activity is to incorporate fun activities into your everyday life. A referral to your local Horizon Leisure Centre will help you to get started in a safe environment and raise your awareness of the exercise opportunities available in the area.

What can I expect from the Exercise Referral Scheme

A personal exercise referral instructor.

A personal activity plan based upon your medical condition, health status, what you enjoy doing, what would you like to try and your lifestyle.

A 12 week opportunity to use the gyms, swimming pools and exercise classes at both Havant and Waterlooville for a reduced fee of £3.50 (gym session only) per session or unlimited use for £49.95.

Regular contact with your exercise instructor to review activity plans.

At the end an opportunity for continued membership at Horizon Leisure Centres.


£49.95 for an initial consultation with an activity plan and unlimited use of both Havant and Waterlooville gyms, pools and classes for 12 weeks

£9.95 for an initial consultation with an activity plan and only £3.50 per gym session for the duration of the 12 week activity plan.

How can I sign up

You will need to be referred by your health professional or GP. You can download the referral form from the downloads section and request to be referred.

Patient Selection Criteria

• Aged 16 years or over

• No absolute contra-indications to exercise

• Any of the medical conditions listed
below in 'Medical Conditions', but no more than 3

Medical Conditions

• Weight Loss BMI  30+ at least one
other risk factor
High Cholesterol
Family History
Physically Inactive

• Asthma (not exercise induced) + at least one other risk factor
Physically Inactive

• Hypertension

• Diabetes

• Mental Health (mild and controlled conditions) including stress, anxiety, depression, post-natal depression and seasonal affective disorder (SAD)

• Arthritis including osteoarthritis and rheumatoid arthritis

• Osteoporosis

• Joint injury rehab (neck, spine, shoulder,
hips, knee, ankle) following suitable advice and exercises from a physiotherapist or similar therapist

• Chronic Fatigue Syndrome / Myalgic Encephalomyelitis (ME)

• Post Phase IV

Absolute Contra-indications

• Unstable angina / heart conditions

• Resting systolic blood pressure 180mmHg or resting diastolic blood pressure 100mmHg

• Significant drop in blood pressure during exercise

• Uncontrolled tachycardia > 100bpm

• Severe or poorly controlled asthma/COPD

• Conditions that may be aggravated by exercise (muscle, bone or joint conditions)
• Uncontrolled conditions including diabetes, asthma, epilepsy

• Ventricular / aortic aneurysm

• Uncontrolled arrhythmia

• Febrile illness

How to refer a patient

• Explain the Horizon Leisure Centres Exercise Referral Scheme to your patient

• Decide which centre the patient would like to attend

• Complete and sign a referral form

• Ask patient to sign the referral form

• Give the patient their copy of the referral form and an information leaflet

• Send copy of the referral form, marked ‘Confidential’ to info@horizonlc.com.

The centre will then contact the patient to arrange their first appointment.
